NDSBRC office hours are 8:00 a.m. to 5:00 p.m. CST Monday through Friday. The office is closed from 12:00 to 1:00 p.m. for lunch and is also closed on State and Federal holidays.

2009 License Renewal

It is that time of year again for licensure renewal. Your N.D. license expires January 31, 2009. You may now renew your license online by clicking the License Renewal link at top of the site. If you wish to renew with a paper application, you may print one by clicking here.

Make sure to have the following information before you begin the online renewal process:

1. Your first and last name as they appear on the renewal letter you received
2. Your social security number (for identification purposes only)
3. Your license number
4. Continuing Education titles and provider numbers
5. A current credit or debit card account
6. The expiration date of the credit or debit card account.

Please call the NDSBRC office at 701-222-1564 if you are unable to log into the online renewal area. This may be because the NDSBRC does not have your social security number on file. The NDSBRC needs this information to verify your license in order for you to view your personal information and renew your license online.

We will be waiving the notarization requirement this year in order to offer the convenience of online services. Your renewal form, renewal fee, and continuing education verification must be completed online or postmarked on or before midnight CST December 31, 2008.

For renewals completed after Dec 31, 2008:

1. A $10 late fee will be assessed in addition to the $60 renewal fee
2. You will be required to show proof of continuing education attendance
3. We cannot guarantee that your renewal application will be processed prior to the January 31, 2009 expiration date.

All requested information must be included for the application to be considered complete. Incomplete applications will not be processed and will be returned to the sender. There is a $25 fee for all returned (NSF) checks and your license will be invalid until all fees are received. Failure to renew or meet the continuing education requirements, as established by the NDSBRC will result in the expiration of your license on January 31, 2009. North Dakota State Law requires persons engaged in the respiratory care and polysomnography practice to be licensed.
