Application for Licensure Information and Instructions

General Information

  1. In accordance with Chapter 43-42 of the North Dakota Century Code, it is necessary for each Respiratory Therapist to apply to practice respiratory care in the state of North Dakota. It will be necessary to renew licensure each year in the manner prescribed by the rules of the North Dakota State Board of Respiratory Care.
  2. It is unlawful to engage in the practice of respiratory care in the state of North Dakota without a license.
  3. All requested information must be included for the application to be considered complete. Incomplete application will not be processed and will be returned to the sender.
  4. The license fee is $60 for a Registered Respiratory Therapist (RRT), Certified Respiratory Therapist (CRT), Temporary Respiratory Therapist (TRT eligible) (Temporary license is ONLY for new graduates waiting to pass the Certifying Examination), or Polysomnographic Technologist (PRT). There is a $15.00 fee for all returned (NSF) checks and your license will be invalid. MAKE CHECK OR MONEY ORDER PAYABLE TO: NORTH DAKOTA STATE BOARD OF RESPIRATORY CARE.
  5. YOUR NOTARIZED SIGNATURE IS REQUIRED!
  6. All out-of-state applicants must send the Licensure Verification to states they have been licensed in. This applies to all states whether your license there is current or has expired.
  7. Effective August 1, 2011, according to NDCC 43-42, all applicants for initial licensure or licensure by reciprocity shall be required to submit to a statewide and nationwide criminal history check

    All costs associated with obtaining a background check are the responsibility of the applicant.

    Once your application for license is received by the Board office, a Criminal History Record Check Request form and two (2) fingerprint cards will be mailed to you. Complete the form and mail it back to the Board office along with the two (2) fingerprint cards containing your set of prints. Both cards must be printed on and submitted directly to the Board office.

    The cost to run the criminal history checks is $47.25. You must submit a check or money order for $47.25, made payable to the North Dakota Attorney General along with the form and both fingerprint cards to the Board office. You may get fingerprinted at your local law enforcement agency. Please call ahead, as fingerprinting may only be offered on certain week days. There may be an additional cost for fingerprinting depending on the agency you go to.

Instructions

All initial applicants (including those applying for a temporary license) must submit the following items to be considered for licensure:
  Completed Application for Licensure.
  $60 license fee.
  Completed Education Verification form.
  Official copy of your transcript sent from the college or university or a copy of your diploma which is
         either notarized or embossed with the school seal.
  Notarized copy of your registering or certifying exam scores.
  Photocopy of BPRT certificate. (If applying for PRT license.)
  Submit to a statewide and nationwide criminal history record check. (Request form and fingerprint cards will be sent to you once your application for licensure is received by the Board office.)

All out-of–state applicants must also submit:

  Completed License Verification form from each state in which you are presently and/or were
         previously licensed.

All requested information must be included for the application to be considered complete. Incomplete applications will not be processed and will be returned to the sender. There is a $25 fee for all returned (NSF) checks and your license will be invalid until all fees are received. Failure to renew or meet the continuing education requirements, as established by the NDSBRC will result in the expiration of your license on January 31, 2012. North Dakota State Law requires persons engaged in the respiratory care and polysomnography practice to be licensed.

NOTICE: A license may be issued within 2 to 5 days from the date all required application materials are received, considering the applicant has no past convictions or disciplinary actions, or there are no other concerns regarding the application.
